Dr. Maria Rodriguez, Founder & Medical Director of the
Community Health Center, launched the clinic in 2015 after seeing
the healthcare gap in underserved neighborhoods. Today, her center provides free
healthcare to over 5,000 families annually and has expanded to include dental and
mental health services.

How our organizations came to life
Every building on the map started with a moment; a need noticed, a conversation started, a bold idea turned into action.
2012 · Riverbend Community Pantry
From one shelf to a full pantry
Started as a single shelf of canned goods in a church hallway.
Volunteers noticed students skipping lunch and quietly built a network of donors,
eventually opening a dedicated space with refrigeration, fresh produce, and evening
hours for working families.
2015 · Community Health Center
Closing the healthcare gap
Founded by Dr. Maria Rodriguez with two volunteers.
What began as a small clinic has grown into a multi site health hub offering medical,
dental, and mental health services to thousands of families.
2016 · Future Path Youth Center
A safe place after the last bell
Launched by teachers who kept their classrooms open after school.
What started as homework help turned into a full youth center with mentorship,
creative labs, and leadership programs designed by students themselves.
2019 · HopeLine Counseling Collective
Making mental health feel normal
Founded by local counselors who wanted walk in, low barrier support.
They created a hybrid model. Drop in hours, group circles, and virtual sessions so
teens and families could get help without long waitlists or complicated paperwork.
